For your convenience, we have created the following pages to provide you with a list of some of the insurance carriers we accept at our Elizabethtown office as well as several forms to save you time at your next appointment. Please click on one of the links below to learn more.

Cancellation/No Show Policy
(Effective September 1, 2016)
In non-emergent situations, we require at least 24 hour notice if you are unable to keep
your appointment. This allows our office sufficient time to offer your appointment time
to our other patients. Failure to provide at least 24 hour notice will count as a missed
appointment and a no show fee may be imposed*. If you are unable to keep your
scheduled appointment due to emergency circumstances, please call our office as soon
as possible to reschedule. Also, as a courtesy to your family, we offer back to back
appointment times for two family members. However, this courtesy requires a larger
block of reserved time and 24 hour notice is necessary in the event you are unable to
keep the appointments. If 24 hour notice is not given, in non emergent situations, a $75
fee per appointment may be imposed and family members will no longer be allowed to
schedule together.

A No-Show fee 0f $75 may be charged, not payable by insurance,  and future appointments may be required to be pre-paid via credit card.
